Why do I have to fill out this very large form prior
to my visit? |

Our staff enters the information you gave
into our electronic medical record prior to your
visit. This way, when you are shown to your
examination room, your time does not have to be
spent entering the information at that time. Our
plan is that you see the physician or nurse
practitioner at, or very near, your appointment
time.

Why
do I have to pay my co-pay and coinsurance at the
appointment instead of waiting for the insurance
company explanation of benefits? |

Our contracts with the insurance companies
mandate our collection of these fees up
front. Also, this decreases costs of billing
to the office.

What
methods of payment do you
accept? |

|
|
We accept cash, credit cards, PayPal, and personal checks.
|

What if you can't verify and confirm my coverage? |

|
|
You will be responsible for paying the full charge at the time of your visit. You may then submit the bill to your insurance company for
reimbursement.

I
understand you give a discount for paying at the
time of service. Is this
true? |

|
|
Yes. If you have no insurance, or we do not
participate with your insurance company, we offer
a 30% discount on our professional fees for paying
at the time of service. We have arranged discounts
averaging 50% on lab fees for those patients
without coverage, so no additional discount is taken.

I don't have insurance. How much will a visit cost ? |

|
|
Your initial visit is more than subsequent visits because of the additional time and attention it takes to review your medical history. After a discount for prompt payment, an initial visit for a problem will typically cost around $130, although it could be more or less depending on complexity. After you are established, the same visit would typically cost around $75. Annual exams cost around $150 (varying by your age at the time of the exam). We have arranged additional discounts for your lab work if you choose to pay us at the time of you exam.

What
do you charge for a returned check? |

|
There is a $25 fee for checks under $50, and a $30 fee for checks over $50 that must be
paid in addition to the amount of the check. Both of these must be paid in cash or bank check. We
regret that any checks not paid for must be prosecuted.

How much do you charge to fill out
forms? |
| |
|
|
All forms incur a $20 charge per
form with a 7 day turn around time. For
expedited, 24 hour service we charge $50 per
form.
|

Can I take my chart to another medical
provider? |
| |
|
|
While the chart itself remains the property of My Gynecologist, PA, we can provide a copy. Per
state law, reasonable copying costs of $1 for the first 25 pages and $.25 for each additional page
are charged. If we can fax these pages instead of printing them, there is no charge.

I have a large
deductible. Can I make multiple payments after my surgery? |

|
Unfortunately, because so many others have not fulfilled their obligations, we must insist on collecting this money before performing any procedures.
|

Do you charge me to send me a bill? |
| |
|
|
The first invoice we send you for any overlooked or misquoted charges is included in our fee. If you
choose not to pay it in full we add a $10 fee to subsequent bills to offset our expenses.

What happens to those people who do not pay their bill? |
| |
|
|
After two attempts to collect the bill these people are turned over to our Collections
Agency. An additional $35 fee is added to their balance and, if not paid, their credit will be
marred with Experian, TransUnion, and Equifax. We also utilize the court system to
enforce these obligations and for these, they are responsible for all additional court costs.

What
do I do if my insurance company sends me the provider's check? |

Please send or bring it to our office along with any accompanying paperwork as soon as possible.

What should I do when
my insurance coverage
changes? |

|
|
Promptly notify My Gynecologist, PA so that we can bill the appropriate company. This will also decrease the chance that the entire balance becomes your responsibility.

Do you make reminder phone
calls? |

|
|
Yes. We have a very robust computerized system that interfaces with our Electronic Medical Record. It makes reminder phone calls two business
days before your appointment. This way, if you have to cancel your appointment, we can try to
fill that spot. Many patients say it is very reassuring to receive a reminder phone call in the
voice of their provider.

Will you leave a message on my answering machine or
voice mail concerning normal lab
results? |

|
|
If you agree in writing, we will do this
under certain circumstances. Sometimes, good
medical care dictates that, regardless of the
results of your lab work, you return to the office
for the results and to ask questions.

Will
you discuss my medical condition, diagnosis,
payment, and/or health-care options with my spouse
or children? |

What is an Electronic Medical
Record? |
| |
|
|
My Gynecologist, PA is at the forefront of
computerized medical record keeping. Our EMR
enables us to access your information more
quickly, and record it more legibly. We never have
to look for a misplaced chart. Laboratory tests
are ordered electronically, and an automatic
reminder system ensures that the results are
imported automatically into your chart, as soon as
they are done. Because the EMR is integrated with
our practice management software, appointment and
billing information is stored with your chart and
is readily accessible.

What do I do if I need a refill of my
medication? |

|
|
If this is a medication that we have prescribed for you previously, please call your
pharmacy and have them fax us a request at 352-684-6888. It can take us up to 2 business days
to respond to this request. If you are requesting a new prescription, or prescription that was
previously given to you by another provider, you must be seen prior to fulfilling this request. We
do not fill pain medications or antibiotics over the phone.

What
happens if I put the wrong information on my
medical history form? |

|
|
Everyone makes mistakes, and overlooking a
small item may be of little consequence. It is
imperative, however, that you are honest in
answering all questions, and do so as completely
as possible.
|

Why
do you request that so many things be faxed to you? |
| |
|
|
Because most of these things can be
inserted directly into your Electronic Medical
Record. We do not have a traditional fax machine.
All faxes are sent and received electronically
through our server.
|
|
What
happens if I'm late to my appointment? |
| |
|
|
Please call us at the first indication that
you may be late. We will work with you to try to
avoid rescheduling. If you are more than 15
minutes late for your appointment however, please
be advised that you will most likely be asked to
reschedule or wait until those patients that are on time have been seen.
|
|
No doctors ever run on
time, so why do you insist that I be on time and have all my paperwork completed? |

Although emergencies (and babies) happen, we do strive to run on time. If patients before
you run late, and we see them anyway, this would inevitably make you late as well, which would not
be fair. Please understand that this policy is for your benefit as well as others.

Do you take care of high-risk
pregnancies? |
| |
|
|
Inevitably, some pregnancies become high-risk after we begin taking care of them.
However, because of the malpractice climate in the state of Florida, we will not accept some patients
for care. When questions arise, these cases are individually evaluated by the physician.

What
can I do to help improve the malpractice situation
in the state of Florida? |
| |
|
|
Write your US Senator. Legislation to improve the situation has passed the House of
Representatives several times, only to be filibustered in the Senate. If it were allowed to
come to a vote, it would likely pass.

What
are your office hours? |

|
|
Our office is open Monday and Wednesday
8:00 a.m. to 7:00 p.m., Tuesday, Thursday and
Friday 8:00 a.m. to 5:00 p.m., and some Saturdays 9:00 a.m. until 1:00 p.m., excluding holidays.

What
do I do if I need something outside of these hours? |

A physician, midwife, or nurse practitioner is available 24 hours a day for emergencies and deliveries only.
Chronic problems, prescription refills, and appointments are not appropriate
reasons to call after hours.

are doctors fees so high? |

Why would you charge me $40 for a missed appointment? |
| |

This fee would not be charged if you advise us 1 business day prior to the appointment time. However, you should know that failure to notify us not only results in lost revenue to the practice, but a lost opportunity for another patient to be seen at that time. |

I don't understand how you can charge that much when the doctor only spent five minutes with
me! |

|
Sometimes, because of their anticipation of their visit, patients feel as if the doctor did
not spend very much time with them. Our doctor and nurse practitioners are very
conscientious, and spend the amount of time necessary to adequately research your problem. It
is a common misconception that the only time the physician spends in helping you is the time that
they are in the room. On the contrary, there are records to be reviewed, phone calls to be made, a
medical record to be written, and a report to be sent to the referring physician. In addition,
there are the everyday business needs and decisions to be made.

Are
you really the only female obstetrician/gynecologist in Hernando County? |

|
We were until late in 2007. Now we are the longest serving female OB/GYN in Hernando County. Other local providers utilize Certified Nurse Midwives, as do we. While
qualified to provide much of the care you may need, they are not physicians, and should not
represent themselves as such. There is also one local female gynecologist who no longer provides
obstetrical services. |
